          MDNY VISION BENEFITS SUMMARY

• One routine eye examination (including dilation) per calendar year (copayment is consistent with your plan benefit, except for split copayment plans where the copayment for specialist services is different than the one for primary care services. Davis Vision is considered a specialty care provider.)
• Up to a 25% discount on Laser Vision Correction procedures.
• Substantial discounts on frames, eyeglass lenses and contact lenses.
• Membership in Lens 1-2-3®, an exclusive mail order contact lens replacement program providing name-brand contact lenses at pricing up to 50% off.

          UTILIZING the MDNY VISION PLAN

Members simply select a participating vision care professional from the MDNY Directory of Physicians and Healthcare Providers and call for an appointment. If the provider prescribes corrective eyewear, or the prescription for existing eyeglasses or contacts has changed, members will be able to choose from a wide selection of fashionable frames, lens options or contact lenses. When the new eyeglasses or contacts lenses are ordered, members simply pay the discounted price. The sections below explain how members can obtain discounted prices. The MDNY vision network administered through Davis Vision has wholly owned full service providers in addition to numerous contracted private practices. To identify a wholly owned Davis Vision Center a Member can look in the MDNY Directory, where Davis Vision will appear above the staff doctors name. If it does not say Davis Vision then it is a contracted private practice.

Service must be received from a Davis Vision Network Provider. There is no reimbursement towards services rendered by an out-of-network provider.

LASER VISION CORRECTION

Laser Vision Correction procedures can eliminate the need for corrective eyeglasses or contact lenses for many people. MDNY members can receive up to a 25% discount on Laser Vision Correction Services (or 5% off an advertised price, whichever is lower) by using one of the experienced eye surgeons in the Davis Vision Laser Vision Correction Network. The MDNY Member Kit contains a listing of participating Laser Vision Network providers.

LENS 1-2-3® - MAIL ORDER REPLACEMENT CONTACTS

The MDNY vision plan also provides free membership in Lens 1-2-3®, a simple, cost-effective way to buy replacement contact lenses at significant savings. With a current prescription, members simply call the Lens 1-2-3® telephone number (1-800-LENS123) and place their order. Contacts will be shipped the same day the order is received at a savings of up to 50% off the retail price.
